How does it talk?

A screen reader comes with an extra piece of software called a speech synthesiser. You do not need to configure this, it loads itself automatically. When the screen reader detects changes on the screen, it sends messages to the synthesiser, and the synthesiser turns these messages into audible speech, which you can hear through the soundcard on your computer.
